Updating registry with
I am 27 now and have never driven a vehicle that was more fun. It has been to the top of Pikes Peak, the Grand Canyon and I climbed Radar Hill at Fort Hood with it. I am just starting to rebuild the little bugger, I will be needing some parts. Nolan [email protected] have two LJ20s both are soft tops.
I would love to find a good running engine to put into it. I was sure excited to find this page - for a long time it was a struggle to find parts and my manual is missing lots of pages. One is original in mint condition I have owned it since 1983 and was lucky enough to buy the last Bestop soft top they had available for the Mini Brute.
Step 3: Mail or fax your completed advance directive, along with your signed Registration Agreement to: Vermont Advance Directive Registry 523 Westfield Ave/PO Box 2789 Westfield, NJ 07091-2789 Fax: 908-654-1919 If you have already registered your advance directive but are looking to make changes or update your document, you will need to complete a new Advance Directive, attach it to the Authorization to Change form and send your new document back to the VADR.
To register the physical location of your advance directive document, rather than the document itself, you may fill out and send the Advance Directive Locator Form instead of a copy of the advance directive to the Vermont Advance Directive Registry.
Registering your advance directive can be accomplished in three easy steps: Step 1: Complete your advance directive form. Step 2: Complete and sign the Registration Agreement.
This gives the registry permission to send a copy of your advance directive and emergency contact information to those authorized to access them.
